Jaremy Mallister is a knight , and member of House Mallister. He is a loyal bannerman of House Tully.

Season 3
Ser Jaremy was among the prisoners of the joint Northern-Riverlands army held captive at Harrenhal. Before abandoning the castle, Ser Gregor Clegane had all the prisoners put to the sword. When the northern army, led by Robb Stark, discovered the corpses, Catelyn Stark recognizes Ser Jaremy.[1]

In the A Song of Ice and Fire novels there is no character named Jaremy Mallister.
Given that he isn't called "Jaremy Mallister" on-screen but just "Ser Jaremy", there is an outside chance that he was simply meant to be a knight in service to House Mallister (explaining why he is wearing the Mallister sigil), while he is not himself a Mallister.
